Before records can be destroyed or erased the bureau has to establish that the physician has another copy, of some form, in his possession. The patient and the physician have the right to know any little alteration on any record. This will help in knowing what information has been erased and what significance it has as far as patient’s medical process is concerned. 6. Yes, all individuals and organizations with some form of access to the computerized databases, and the level of access permitted should be specifically identified in advance. Full disclosure of this information to the patients is necessary in obtaining consent to treatment. patient data should be assigned a security level appropriate for the data’s degree of sensitivity, which should be used to control who has access to the information. The patient has the right to know who have an access to his/her information and why. This will for the respect of the patient’s right to privacy and confidentiality. 7. Does the AMA ethics opinion mention encryption as a technique for security? Yes, the computerized data systems have a compromising information security. The (AMA) opinion is that â€Å" there should be controlled access to the computerized database via security procedures such as encryption (encoding), passwords, and other user identification including scan able badges†. Confidentiality agreements should be made with other healthcare professional whom the office networks with encryption is recommended if the network entails public channel of communication such a radio waves, telephone wires, and microwaves. This will increase the changes of information confidentiality. 8. In regard to electronic medical record (EMR), what is the policy for disclosing authorized data requested by third parties? The patient must give consent in writing authorization for disclosing any information about his/her medical record. the individual or groups requesting the data required to obtain the expressed consent of the patient. The dissemination of confidentiality medical data should be limited to only those individuals or agencies with a bona fide use of the data. Motivation theories There are many theories at motivation given by the different authors that are elaborated below to catch the research topic in depth. The idea that all people have inner driving forces that motivates the people promptly to do certain activities. This is very old idea, in use long before the appearance of what we now call theories of motivation. There was the Hawthorne Studies (1939) which gave new thinking to the new school of management thinking, the human relations movement. The most ideas about it, are that the people have social needs that are as vital as the economic imperative. There are two groups of the theories known as process theory and content theory that are further extended. Content theory assumes that people strives to satisfy of deep rooted needs. While they differ in terms of their assumptions about the relative importance of different needs, it is the craves to satisfy them that is said to energize behavior. Maslow needs theory Maslow work (1954) is based on the assumption that human needs are inexhaustible. As one set of the needs is satisfied the anther set of need come to open in its place which means that needs are arranged in a hierarchy as shown below in the form of pyramid of five levels. Here physiological needs are most basic of all and arise from internal physical imbalance such as hunger, thirst, warmth and shelter. Security needs are promoted by the strong and hard social nature, freedom from pain or destruction, emotional security and well being. Affiliation needs are promoted by the strong security nature. As most of the people enjoy feeling of love, like relation and support from others. Maslow theory has a number of highly important assumptions, two of which have significant implication for the motivation. He argues that different level of needs are universally addressed sequentially that mean the needs are one level will not normally play a vital part in motivation. Strong issues The idea that the hierarchy is universally applicable no account of cross culture difference. It is patronizing and elitist in term of the values it expresses. Maslow ideas have a mystic, metaphysical quality that is largely the result of armchair theorizing and often contradicted by the research evidence. Alderfer ERG theory In 1972, he also introduced the idea of hierarchical ordering but this has only three levels. Existence Relatedness Growth In his theory, different levels are viewed more as a continuum than as discrete categories. He does not measure a sequential progression up the hierarchy, but permits for more than one level to be open or active at same time. Although he suggested that satisfaction of needs at one level will lead to the someone seeking satisfaction at the level above. Issue Alderfer theory has got little empirical testing beyond that done by its author. He argues that the theory could be a more powerful but empirical elaboration of the effects of needs than Maslow ideas (Alderfer 1972). Herzberg two factor theory Motivators link to the factors which are intrinsic in nature like appreciation for a job task done is highly motivating. While hygiene factors referred to extrinsic forces e.g. relationship with colleagues which dont actually relate to the employees actual job. According to Mullins (1993) intrinsic or motivators factors are attach to work content and lack of intrinsic or motivators factors doesnt demonstrate extremely de-motivating or dissatisfying. For that reason, these elements are expressed as motivators or satisfier. The hygiene factors linked to the work conditions. These factors do not direct to the enlarged satisfaction or work participation though, if these factors are not found or absent then the effects will be de-motivation and dissatisfaction for example low wage rates. Because these factors are essential to the extent of no de-motivation or dissatisfaction these are called the hygiene or dissatisfiers factors (Leopold, 2002). According to Rainey (1999) the hygiene or dissatisfiers are the factors which are not directly concerned in motivation of employees but its interesting that these factors are important to give a sound base to beat the employee dissatisfaction and de-motivation. However, its not essential that if we improve the hygiene or dissatisfiers factors that result in motivation. Herzberg represented a theory in the light of employees performances and attitudes which is called Herzbergs two factor theory. Herzberg recognized two categories of factors which influenced the work and motivation. First category of sets called hygiene factors and the other set of factors called motivators factors. Hygiene factors contain employment security, pay, organizational policy, status and interpersonal working relationships. Absence of hygiene factors cause dissatisfaction and de-motivation at work place. Motivator factors include success, development, nature of work it-self, respect, recognition and responsibility. Absence of motivator factors caused motivation and satisfaction (Leopold, 2002). Figure 1 is the pictorial form of these factors as followings. Issues There are some issues allocated by some researcher about Herzbergs theory that the finding gathered from a small number of samples can be generalized to the whole industry. For instance, the outcomes from the research on a small engineering unit could be applied to other professions or not. Other scholars are not happy with the Herzbergs methodology and procedure followed for conducting the research because that research methodology involved individual to consider the critical incidences from their past which make them either delighted or sad. Some other researcher are also not in his favor that Herzbergs theory was not give any clear justification that why some intrinsic and extrinsic factors motivate or satisfy the individual and why influence their work (Rousseau,1997). Extrinsic and intrinsic motivation Employee motivation at work can be split up into two kinds on the behalf of classic literature and practical research as one is extrinsic motivation and second is intrinsic motivation (Donovan, 2001). Extrinsic motivation is a form of motivation where employees fulfill its needs and desires through indirect means particularly through the financial compensation, In point of Thomas (2000) view. On the other side, intrinsic motivation come into action when employees behavior is inclined towards the fulfillment of natural psychological desires and needs rather than to attain material incentives (Marks, 2001). In all form of organizations small, medium or even large both extrinsic and intrinsic types of motivation can motivate and satisfy employees to perform their job at work place. However, extrinsic and intrinsic motivations both have very diverse effects on the individual mind-sets, feelings to job performance, passion to complete work and level excellence for performance (Amabile, 19 98). Hoar Rebecca (2004) said that even though the extrinsic motivation can effect in opposition direction to the intrinsic motivation but it also have an emphasizing effect on the intrinsic motivation. Illness Among the Chickens The outbreak began in early June, following the first major rush of tourism for the summer. The streets were crowded with tourists--many American, but some Japanese, Koreans, and Europeans as well. As the townspeople focused on selling their wares to the tourists, none noticed the lethargy of their poultry. As I moved passed my neighbor's poultry shed, headed toward the street to sell my copied music cds (for I am rather well off, having moved to this neighborhood to care for my ailing parents), I noticed that the chickens were lethargic, and had not seemed to have eaten. My neighbor cursed them to me when I saw him, claiming that what few eggs they laid were soft and broke when he picked them up. I moved when he kicked one, and set up my shop for the day. The next day, as I passed the chicken shack, I noticed one chicken lying next to the shack--its face was swollen, and the combs of the cocks had taken on a blueish coloration. The chicken also seemed to be sneezing, but I can never tell. I did not see my neighbor that morning, and I moved on. When I returned home, however, I saw him standing in his yard, staring down. He was flushed, and sweating, but in the heat, I took it for granted. One thing however, stopped me cold--all around him, his chickens were dead. It seemed to have come suddenly--some were dead in the water trough, as if they had been struck down while drinking. After putting on my gardening mask (I have terrible hayfever), I helped pile the corpses into a wheelbarrow and carry them off out of the town to bury them. My neighbor spit and cleared his throat a lot, and complained of a headache and said his arthritis was acting up. I gave him some aspirin, and went home for the night. Several days passed as normal, and I did not think too much of the chickens. After all, this was the busy season, and I had to make sure that I made enough to last my sister a year, when I left her [to watch] my parents [after] I returned home. However, when my neighbor's daughter crawled to my door and passed out, I knew that [the illnesses of my neighbor and his chickens] were somehow related. Information Relevancy for Online Consumer Decision-making Essay

Introduction The emergency of Internet has largely changed the travelers’ decision-making process (Buhalis, 2007) and websites have become such an integral component of today's businesses that website effectiveness are of strategic importance to companies at large and hospitality and tourism businesses in specific. It has been a customary practice for hospitality and tourism companies to conduct business transactions to market themselves (Buhalis, 2003; Buhalis & Law, 2008; Wang, 2008) and to enhance communication between suppliers and consumers (Law, Qi, & Buhalis, 2009). As a result, website effectiveness has been a topical area that received sustained scrutiny from tourism and hospitality researchers. However, only a moderate level of success has been achieved (Law, Qi, & Buhalis, 2009). Recent literature in tourism website evaluation have attempted to investigate this area from various angles including website design and functionality, consumer hedonic experiences and perception of information quality (Law & Bai, 2006; Law, Qi, & Buhalis, 2009). However, only limited research has been conducted in evaluating online travel agencies’ (OTAs) websites and travel agencies’ websites (Kaynama & Black, 2000; Chung & Law, 2003; Roney & Ozturan, 2006). The majority of existing studies in this area are conducted in the context of hotels or tourism destinations. Methodologies employed by these studies are mainly using survey questionnaires to measure the overall satisfaction level of online travelers, or other exploratory approaches such as content analysis or attribute/functionality counting against a predefined checklist (Law, Qi, & Buhalis, 2009). It should be noted that most of the existing research tended to only measure the o... .../desire/action to make final booking. 2) Questions related to their perceptions how relevant the information presented at each webpage is in assisting their decision-making. 3) Questions related to their personal information. Expected Contribution Providing relevant and timely information at each page of a website can be crucial in supporting consumer decision making and therefore website effectiveness. It will potentially provide recommendations to OTAs for online information optimization. Osteoarthritis can also result from diseases in which there is softening of the bone, like Paget's disease in which the long bones of the body curve like a bow, or osteoporosis with its bowing of the shoulder called "dowager's hump," or other bone degeneration. Other forms of arthritis can also cause a secondary osteoarthritis. Osteoarthritis is not an inevitable problem of aging. Those who don't suffer from it may have their heredity and possibly the strength of their immune systems to thank. Medical science is not quite sure of all the factors that com e into play in deciding who gets osteoarthritis and who doesn't. Rheumatoid arthritis is an inflammatory arthritis. It is second only to osteoarthritis in the number of its victims. It affects primarily the small joints in the hands and feet and the synovium, causing crippling deformities. This is an arthritis that usually starts in middle age or earlier. Estimates of the incidence of rheumatoid arthritis run as high as one person in every hundred, and females are two to three times as likely to suffer from it. It seems to start more in the winter and after some siege of sickness, but it is not considered an infective arthritis. Nobody knows what causes rheumatoid arthritis. There may be some hereditary trait, and there seems to be some connection to viral infections like German measles and serum hepatitis, the liver disease brought on by an injection of one kind or another. Because of this, scientists theorize that rheumatoid arthritis may be an autoimmune disease, one in which the body acts as though it were allergic to itself. The immune system gets mixed up and attacks normal joint tissue instead of the stuff it is supposed to attack. Polyarteritis Nodosa is also an inflammatory arthritis, fortunately it is a rare form of arthritis. It can lead to complications that are dangerous to life. It affects four times as many males as females, mostly young adults.

Memories of my Childhood Days My first recollection of my childhood is the day I down the stairs when I was three years old. I was so happy that time I cried and cried for nearly an hour in spite of my parents’ attempts at comforting me. Finally, I quietened down when I was given ice- cream and chocolates. Luckily, I was not hurt because the stairs is not to steep. My days were happy ones before I started going to school, as I had good neighbours to play with me.I was playing from morning till evening. We played games outdoors like Batu Seremban,Police entry and Lumba Lari. There were also other games to occupy our time until my mother bring a rattan to call me back.. At the age of five years, I had to attend kindergarten and this turned out to be quite an ordeal for me. To begin with, I felt very sad having to leave my mother and go off each morning to be amongst strangers. I cried and wanted my mother around, much to my teacher’s displeasure. As the days went by, however, I made friends easily in class and found company. I became happy again and actually looked forward to leaving the house each morning. I still remember those days when I was learning to ride a bicycle. I fell and bruised myself several times but I never gave up. Finally, I was able to join my friends going around the neighbourhood on my bike. Another fond memory is the end-of-year concert held at the kindergarten. I was chosen to play the role of Princess in the play, ‘Princess and Gunung Ledang’, and I became a celebrity overnight receiving lots of compliments for my performance. I now realise what a wonderful childhood I have had and at times wish I could relive those days.

Opening a sports shop in Manchester To begin with we need a place for all the money to go so that we can keep track of incoming and outgoing money so we need a business account with a bank. A bank is a business that provides a place for you to hold your money securely and also providing a way to track and manage your money and accounts making it easier for you to manage your taxes sales and expenditures and having an account you need someone to manage these for you so you would need an accountant there are businesses that can provide you with an accountant or accountants that will work for you they manage your accounts balancing your sales, expenditures and your taxes giving you more time to work on other areas of the business. A lot of business is now done on computers or if it’s a shop on tills so this requires a place for you to get your computers, tills or other hardware needed to run the store being a sports shop we will be using tills for the sales of products, computers for managing accounts and emailing, printers for making bank notes, receipts and bills and telephones for making calls to make sales and orders so we will need a phone company to provide us with a phone line and an account with them for phone usage, an internet provider to give us an internet connection so that we can send e-mails, check accounts online, make a website to advertise and even sell our goods on which will require a web designer who will make the website and manage it for us. We also need electronics stores that could sell us the computers, telephones and printers. Tills are a specialist item to buy so you will need to find a company that will provide and perform the necessary upkeep on them and we also need I.T specialists who can provide us with the software and management of the software for the tills and computers. We also need to find a supplier to provide us with the sportswear and sporting goods for us to sell a supplier is a company that usually make the goods themselves or buy it from the manufacturers and sell everything in bulk for a cheap price and then you sell the goods on for a profit, so they are two more businesses that can help us manufacturers and  suppliers without these businesses we would struggle to get the goods we need to sell. Then we need logistics companies to transport these goods to us and even from us to the customers, we might also need a place to store our goods so we will need warehousing and possibly security services to make it safe to store the goods so you might look into security companies/businesses that will manage the shops and warehouses. We also have accidents at work in stores and possible needs for legal action so we could do with lawyers/solicitors so finding a law firm that can help is also a good thing to be aware of. When you have problems with the buildings that you are using for your business activities you will need trades people to fix any issues you may be having i.e. Say your plumbing is leaking or toilet facilities or malfunctioning you will need to find a plumber to come and fix the problems you are having, if you need lighting or sockets or general electrical work doing you will need an electrician, if you are building a new building you will need architects, brick layers, window fitters, roofers, carpenters, electricians, plumbers, landscapers and possibly more to make your new building. If you want to expand your business and help it grow but you haven’t got the money for it straight away you might consider looking for investors who will give you the money upfront for your business venture for a percentage of the company profits. We also have accidents and tragedies that happen and could cause problems or losses within a company and for these times you can also get insurance from an insurance company who will help you and even pay for any issues that need to be fixed and solved. So to sum it all up there are many businesses that we need to make a business and to keep a business running and without these different businesses there would be no retail and no business.
